Read MorePublished in the Journal of Pharmaceutical Care & Health Systems, the article describes health care disparities which continue to exist in Milwaukee, Wisconsin. Underserved groups experience a higher incidence of cardiovascular disease, other chronic comorbidities, and associated risk factors. Pharmacists have the training and ability to perform physical assessment, conduct screening tests, and educate patients on the prevention and treatment of many disease states. The objective of the study was to develop a pharmacist-led, community-based health screening service to address health disparities through academic-community partnerships. The School of Pharmacy is collaborating with Next Door, an early childhood education and social service organization in the Metcalfe Park neighborhood of Milwaukee, to create a unique, pharmacist-led, community-based health screening service. Through this program, School of Pharmacy faculty and students will deliver an array of primary and specialized health screening services at a clinic location inside Next Door. A Crucial Conversation is a discussion where the stakes are high, opinions vary, and emotions run strong. Learning objectives for the participant: Create a culture of candor where people are able to come to agreement about difficult changes, talk openly and honestly with people no matter how delicate the topic or powerful the individual. Make decisions in a way that encourages people to act on them with conviction; eliminating resistance and negativity. Speak up when things are getting off track (in or out of meetings) and suggest how to make improvements. Effectively hold conversations in which stakes are high, emotions are strong, and opinions differ.
